Official Transcript Requirements
Academic education is verified solely through the review of official transcripts.
- Do not submit unofficial or incomplete transcripts as this delays the processing of all applications
An official transcript must include all items below:
- Verification that academic requirements have been met, as required
- Date of high school graduation or equivalent awarded
- College/university coursework with credits earned
- Date degree was conferred
- Seal of the academic institution
- Signature of the Registrar
Academic Coursework Requirements
Important Information
- All academic requirements are a minimum requirement, so a higher degree can be used to verify eligibility; see examples below:
- If a baccalaureate degree is required, a master's degree or a Doctorate would meet that minimum requirement
- If high school graduation is required, an associate's degree would meet that minimum requirement
- Acceptance into a program or institution does not verify high school graduation or equivalency
- College coursework, without a degree conferred, does not verify high school graduation or equivalency
- Only send official transcripts that are necessary to verify that the academic requirements have been met
- Do not send unofficial or incomplete transcripts
- Multiple transcripts may be submitted from different institutions
- Transcripts from laboratory training programs are not necessary unless the transcript is verifying conferral of a degree; see NAACLS, CAAHEP, or ABHES Accredited Programs for complete details
