Documentation Guidelines 

To apply for an ASCP BOC credential examination, you must verify that you meet specific academic, training, and/or experience requirements, based on the category and route of application. Before you submit your application, review the documentation guidelines to ensure efficient processing.

Documentation Submission Rules 

All documentation must:

  • be in English
  • contain the name of the applicant as it appears on their examination application
  • contain original, handwritten signatures; electronic signatures are not acceptable
  • be PDF documents; pictures are not acceptable
  • be uploaded as individual PDF documents in the online application; documentation sent via mail or email will not be reviewed or processed
  • be no larger than 10 MB

Important Information

  • Requests for testing accommodations must be submitted before applying
  • Applications may be started and "saved for later" for up to 30 days
  • All applications are processed in the order they are received
  • Documents submitted to resolve deficiencies are processed in the order they are received
  • Every documentation form must be submitted with a Letter of Authenticity
  • Multiple documentation forms and Letters of Authenticity may be submitted to verify completion of eligibility requirements
  • All transcripts must be sent directly from the educational institution
  • All foreign transcript evaluations must be sent directly from the evaluation agency
  • All documentation submitted with an application is subject to audit to verify authenticity; this includes transcripts, foreign transcript evaluations, documentation forms, and Letters of Authenticity
  • All documents submitted become property of the ASCP BOC

Already ASCP BOC Credentialed

  • If an ASCP BOC credential is required to meet eligibility requirements, it must be valid (in an active CMP cycle)
  • If the ASCP BOC has a legible, official transcript or foreign transcript evaluation already on file, you may not be required to submit an additional transcript; the ASCP BOC reserves the right to request an official transcript be submitted for any application

Verify Experience or Training

If required for the category and route under which you are applying, training or laboratory experience must be completed and documented before submitting your application. The appropriate documentation forms for the category and route of application must be submitted.
