All training and experience documentation forms must be submitted with a Letter of Authenticity. The same person must complete and sign both the documentation form and the Letter of Authenticity.
A Letter of Authenticity must:
- Be completed by personnel authorized to verify an applicant's training and/or experience (e.g., program official, manager, supervisor, employer, etc.)
- Be submitted on institutional letterhead
- State that the person submitting the Letter of Authenticity also completed the documentation form
- Contain a handwritten signature (not typed or electronic)
- Contain a date that matches, or is after, the date on the documentation form
