Structured Programs

Phlebotomy, Medical Laboratory Assistant, and Histology Programs 

If your program is a formal, structured training program that is not accredited by NAACLS, your students must submit a documentation form and a letter of authenticity to verify their training. 

 

Documentation Forms

Please complete the training documentation form and provide a letter of authenticity printed on your institutional letterhead stating that the documentation form was completed by you. The form and letter must be completed, signed, and dated by the program official. 

Academic Requirements

All applicants must be able to verify that they meet all eligibility requirements, including proof of high school graduation or equivalent. Official high school (or equivalent) transcripts must be sent directly from the educational institution to the ASCP BOC.

  • Acceptance into a program or institution does not verify high school graduation or equivalency.
  • College coursework, without a degree conferred, does not verify high school graduation or equivalency.
