Verify Training

If required for the American Society for Clinical Pathology Board of Certification (ASCP BOC) category and route of application, laboratory training must be completed and documented before submitting your application.

If you have completed a NAACLS, CAAHEP, or ABHES-accredited program, go to that webpage for details.

Otherwise, to document your laboratory training, you must:

  • Download and print the appropriate documentation form(s) for your category and route of application; see Explore Credentials
  • Have program personnel complete and sign the documentation form
  • Have program personnel submit a Letter of Authenticity

Completed documentation form(s) and Letter(s) of Authenticity must be uploaded as individual PDF documents in the online application. Refer to the ASCP BOC Documentation Guidelines for complete information.

As part of your training program, clinical requirements must be obtained at an acceptable laboratory as defined in the eligibility requirements:

ASCP Credential Categories

  • Acceptable clinical laboratory:
    • CMS CLIA certificate of registration, compliance, accreditation;
    • OR DoD CLIP certificate of registration, compliance, accreditation;
    • OR JCI accreditation;
    • OR Accreditation under ISO 15189

ASCPi Credential Categories

  • Clinical laboratory accredited by JCI, CAP, under ISO 15189, or authorized by a governing regulatory association or Ministry. Countries without a prevalent system of accreditation must have laboratories approved by an International Advisory Board appointed by the ASCP Board of Certification.
  • Training programs that do not grant university credit as part of a Medical Laboratory program will not be considered for training verification but will be counted as experience.
