Society of Hematopathology Quarterly Case: De Novo B-Lymphoblastic Leukemia/Lymphoma with MYC rearrangement and surface light chain expression

Intended Audience: Pathologists, Pathology residents, and Laboratory Professionals

This is a case presented in an electronic format to an audience consisting of pathologists, mainly hematopathologists with the goal to familiarize them with a case of B-acute lymphoblastic leukemia/lymphoma (B-ALL) with some features overlapping those typically associated with mature B cell lymphomas. The presentation will include data on the clinical history of the patient, images from the submitted specimen, the diagnosis and a short discussion of the diagnostic criteria and of the differential diagnosis.

Upon completion of this course you should be able to:

·         Understand how to integrate morphologic, immunophenotypic and cytogenetic findings to distinguish precursor B-acute lymphoblastic leukemia/lymphoma (B-ALL) from mature B-cell lymphomas.

·         Recognize that some B-ALL cases may have morphologic, genetic and immunophenotypic features mimicking mature high-grade B-cell lymphomas, such as "starry-sky" appearance, MYC rearrangement and immunoglobulin light chain expression.

·         Understand the importance of TdT expression in the diagnosis of B-ALL
