Please follow the instructions below to submit your answers to the documentation questions for the LabQ and/or CheckSample programs:

LabQ

1. Go to www.ascp.org and log in.
2. If your LabQ was ordered through your institution, please follow the instructions under “Check Sample” below.
3. Go to the “My Courses” link in the red box, upper right.
4. Click the Product Code for the exercise you wish to access.
5. Click the “Exercise” tab, upper left.
6. Click on the link for the Exercise, and read the pdf.
7. Click the “Exercise” tab, upper left.
8. Click the link for “Exercise Assessment,” take the quiz, and click “Continue.”
9. Complete the Evaluation.
10. Follow the prompts to download your certificate.

CheckSample

1. Go to www.ascp.org and log in.
2. Go to the “My Courses” link in the red box, upper right.
3. If your desired courses fail to appear in this area, you will need to associate yourself with your institution, as follows:
   • Go to the “Contact Info” link in the red box, upper right.
   • Click on the “Company Lookup” button under “Business Address.”
   • Enter your order number, search, and select the proper company. You may edit this information as needed.
   • Once you have verified the information, click “Submit.”
4. Go to the “My Courses” link in the red box, upper right.
5. Click on the Product Code for the exercise you wish to access.
6. Enter your Order Number (if necessary, please see your program administrator for this information).
7. Click “Submit.”
8. Click the “Exercise” tab, upper left.
9. Click on the link for the Exercise, and read the pdf.
10. Click the “Exercise” tab, upper left.
11. Click the link for “Exercise Assessment,” take the quiz, and click “Continue.”
12. Complete the Evaluation.
13. Follow the prompts to download your certificate.
