ASCP Board of Certification, an approved certification organization by the California Department of Public Health, offers a certification examination in phlebotomy, Phlebotomy Technician, PBT(ASCP). Examinations are administered on computer year round by appointment.

The BOC Licensure Process

Meet the eligibility requirements.

Submit required documents with a non-refundable application fee to the ASCP Board of Certification office:

• Completed PBT application form (PDF) or complete online application form.
• Completed PBT phlebotomy reference form (PDF) for California Approved Phlebotomy Programs.
• Release form (PDF)

Receive an Admission Notice from the ASCP Board of Certification with instructions to call and make an appointment for examination on the computer at the testing location of your choice. (This Admission Notice will be mailed to you only when you have been determined eligible for examination.)

Study for Exam. View study materials.

Take the examination. Immediately after you complete the examination, you will see your pass/fail result on the computer screen.

Receive your examination scores within ten (10) business days of the date of your examination. Your scores will also be mailed to the California Department of Public Health if you completed and mailed the release form to our office.

Receive your Certificate verifying your certification—that is valid for three years, and the Certification Maintenance Program packet to maintain your certification.
