Application Fee for Medical Laboratory Technician (MLT): $195
Application Fee for Medical Technologist (MT): $220
Application Fee for Cytotechnologist (CT): $220

The ASCP Board of Registry examinations for Medical Technologist (MT) and Medical Laboratory Technician (MLT) have been approved for licensure purposes by the state of New York effective December 1, 2007. The ASCP Board of Registry examination for Cytotechnology was approved September 1, 2007. Applicants for New York licensure must first apply to the state of New York and follow the state application process. Information on the requirements and application process may be obtained at the New York state web site.

Note: There is a six-year "look back" period, meaning that an individual already certified as a Medical Technologist, MT(ASCP), a Medical Laboratory Technician, MLT(ASCP) or a Cytotechnologist, CT(ASCP) after September 1, 2001 does not need to retake the examination.

Please review the steps below for the following options:

  • Option 1: For New York licensure only
  • Option 2: For New York licensure and ASCP certification
  • Option 3: If you were ASCP certified in the appropriate category prior to September 1, 2001
  • Option 4: If you were ASCP certified in the appropriate category after September 1, 2001

Option 1: For New York Licensure only

You must meet the licensure requirements established by the state of New York and successfully pass the appropriate ASCP Board of Registry examination (MT, MLT, CT).

Contact the New York web site for information on completing the licensure process and submit an application to New York.

When you have been determined eligible for licensure by the state of New York, you will receive a letter from the state.

After you receive this letter, complete the ASCP State Licensure Application (PDF) from the ASCP Board of Registry and mail this form with a non-refundable application fee to the ASCP Board of Registry office.

You will receive an admission letter with a phone number for scheduling your appointment to take the ASCP examination. Examinations are administered year-round on computer by appointment at 14 test sites located in New York. You may schedule an appointment to take the examination at your convenience within the assigned 3 month examination period.

Upon completion of the examination, your results (pass or fail) will appear briefly on the computer screen. A written copy of your examinee performance report will be mailed to you that you will receive within ten (10) business days. A copy of your results will also be sent to New York.

Option 2: For NY Licensure and ASCP Certification

You must meet the eligibility requirements established by the ASCP Board of Registry and the licensure requirements established by the state of New York. You must also successfully complete the ASCP Board of Registry certification examination administered after September 1, 2001 for MT, MLT or CT.

Contact the New York web site for information on completing the licensure process and submit an application to New York.

Review the eligibility requirements for the appropriate examination category (MT, MLT, CT) to be sure you have met these requirements.

To apply for ASCP certification, complete the general application (PDF) for ASCP Board of Registry Certification (DO NOT COMPLETE THE STATE LICENSURE APPLICATION) and submit this form with a non-refundable application fee to the ASCP Board of Registry office. An official transcript verifying the degree and science requirements must be submitted and reference forms (depending on the examination route under which you are applying) may be required. Please refer to the Procedures for Examination and Certification (PDF) for detailed eligibility requirements. Sign and submit a release form (PDF) authorizing our office to submit your examination scores to New York.

Upon completion of the examination, your results (pass or fail) will appear briefly on the computer screen. A written copy of your examinee performance report will be mailed to you and a copy of your results will be forwarded to New York within ten (10) business days after we have received your final transcript verifying completion of the academic requirements. Upon passing the examination, you will receive a certification number on your examinee performance report and a certificate verifying your certification within 6-8 weeks.

Option 3: ASCP Certification before September 1, 2001

It will be necessary for you to retake and successfully complete the appropriate certification examination (MT, MLT, CT). If you do not pass the examination, it will not affect your certification status.

You may schedule an appointment to take the examination at your convenience within the assigned 3 month examination period.

Upon completion of the examination, your results (pass or fail) will appear briefly on the computer screen. A written copy of your examinee performance report will be mailed to you that you will receive within ten (10) business days. A copy of your results will also be sent to New York.

Option 4: ASCP Certification after September 1, 2001

You do not need to reapply or retake the ASCP Board of Registry certification examination.

You will need to follow the instructions under Verification of Certification to have your examination results sent to the state of New York to verify successful completion of the ASCP Board of Registry certification examination.

Choose the option to send your examination results to a third party and indicate the email address provided in the letter sent to you from the state of New York.
