The BOC Certification Process
Congratulations on your decision to apply for certification by the ASCP Board of Certification! The ASCP Board of Certification (BOC) is a recognized leader in certification of medical laboratory professionals. By earning credentials from the ASCP BOC, laboratory professionals demonstrate their competence to carry out their responsibilities in this critical profession. In turn, they reap the benefits of better job prospects, higher salaries and the respect of colleagues. Long considered the “gold standard,” BOC certification is sought out by seven times as many laboratory professionals as any other lab professional credentialing organization.

Let's get started. Please read and follow the instructions under the appropriate certification category listed below, as well as in each of the tabbed sections here in order to properly complete the entire certification process.

Please review all of the following before submitting your application for the Cytotechnologist, CT(ASCP) certification examination.

Eligibility and Documentation
Eligibility
Provided they are in good standing, active military personnel currently enrolled in a CAAHEP accredited Cytotechnology program or who have completed a CAAHEP accredited Cytotechnology program within the last 5 years can sit for the ASCP BOC Cytotechnology, CT(ASCP) certification examination as described below.

Applicants may apply under the following route:
Route 1: Baccalaureate degree or higher from a regionally accredited college/university AND successful completion of a CAAHEP accredited Cytotechnology program within the last 5 years.

Documentation
Applicants should collect the required documentation to verify their training and academic education as follows:

1. If your program is CAAHEP accredited - contact your Program Director for the school code number. This number must be indicated on the application form.

An official college transcript verifying a baccalaureate degree must be submitted following graduation to the ASCP BOC office at 33 W. Monroe St, Suite 1600, Chicago IL 60603. Contact your program director if you have any questions regarding your college transcript.

2. If you have already successfully completed a CAAHEP accredited Cytotechnology program within the last 5 years - indicate the school code number on the application form.

An official college transcript verifying a baccalaureate degree must be submitted to the ASCP BOC office at 33 W. Monroe St, Suite 1600, Chicago IL 60603.

Applying for Certification and Application Processing
Applying for Examination
Submit your application. The application should be completed, following the instructions on the application form. It is imperative that your personal email address is included since all information will be sent by email. Your full, complete mailing address must also be included for mailing of your certificate, if you pass the examination. It should be signed and dated. Please note: application fees are non‐refundable.
- Payment by the U.S. Military by credit card - If your application fee is being paid by the military, submit your completed application form to the appropriate military personnel to be submitted to the ASCP BOC office. When payment is received from the military and authorized, processing of your application will begin. Complete processing of your application will take 30-45 days from the date payment is authorized.
- Payment by the U.S. Military by check- If the program director submits application forms for their students along with one check to cover all application fees, these applications, the check, and any accompanying documents should be mailed to the following address: ASCP Board of Certification, 3462 Eagle Way, Chicago, IL 60678. This will ensure that the payment is correctly allocated to the students’ applications.
- Personal Payment - The fastest method for submitting the application form is online with a credit card. You may choose to print the application form and mail it with a check. Printed application forms with checks or money orders are mailed directly to the bank processing center (3335 Eagle Way) for deposit, and then forwarded to the BOC for review and distribution to the evaluator. Allowing for postal mail delivery, bank processing time, and delivery to the BOC office, this method may take up to 15 business days for the application to reach the BOC office. Because the bank processing center handles a high volume of mail per day from many different companies, applications should not be sent by certified mail, FedEx, UPS, or any type of delivery service except regular U.S. postal mail to avoid further delays. Complete processing of applications takes 30-45 days.
Transcripts or any correspondence without a check or money order should be mailed directly to the BOC office at: Board of Certification, 33 W. Monroe St., Suite 1600, Chicago, IL 60603. If sent to the bank address above, these documents will be bundled with other miscellaneous documents, and then forwarded to the BOC office.

Applications for certification examinations are processed immediately as they are received. As soon as the program director signs off on the form emailed to them (for current students in CAAHEP accredited programs only) the applicants will be notified to access their admission letter, barring any other issues with the application. Applicants who have completed a CAAHEP accredited Cytotechnology program within the last 5 years will be notified to access their admission letters as soon as all appropriate documentation, as described above, is submitted. During peak volume times from May through August, there may be a delay in emailing admission letters, but complete applications are processed within a 30-45 day period. When eligibility has been established by the ASCP Board of Certification, the candidate will be notified of the following:
- Admission letter
- A three (3) month window of eligibility.

Examination Results
Scores for students enrolled in a CAAHEP accredited Cytotechnology program will not be released until an official transcript documenting completion of the academic requirements is received in the Board of Certification office. Scores for candidates who have already completed a CAAHEP accredited Cytotechnology program within the last 5 years will be available four business days after they have taken the examination, provided all official transcripts verifying the appropriate degree have been received. Applicants may apply under the following routes. Choose the route that applies to you.
Route 1: Successful completion of a NAACLS accredited Histotechnician program within the last 5 years prior to the date of application for examination.

Route 2: At least 60 semester hours (90 quarter hours) of academic credit from a regionally accredited college/university, with a combination of 12 semester hours (18 quarter hours) of biology and chemistry (must include credit hours in both), or an associate degree from a regionally accredited college/university, with a combination of 12 semester hours (18 quarter hours) of biology and chemistry (must include credit hours in both), AND one year full time acceptable experience in a histopathology (clinical, veterinary, industry or research) laboratory in the U.S., Canada or an accredited laboratory* within the last ten years.

*Laboratory accredited by a CMS approved accreditation organization (i.e., AABB, CAP, COLA, DNV, The Joint Commission, JCI, or under ISO 15189.

Provided they are in good standing, active military personnel currently enrolled in a 50-week U.S. military laboratory specialist course, or in a NAACLS accredited medical laboratory technician training program can sit for the ASCP BOC Medical Laboratory Technician, MLT(ASCP) certification examination within the last three weeks of Phase II training. Military personnel who have already successfully completed the 50-week U.S. laboratory specialist course/NAACLS accredited program and the required education may also apply for certification. The application process is described below:

Applicants may apply under the following routes. Choose the route that applies to you.
Route 1: An associate degree or at least 60 semester hours (90 quarter hours) of academic credit from a college/university accredited by a recognized regional or national accreditation agency, AND successful completion of a NAACLS accredited MLT program within the last 5 years.

Route 3: An associate degree or at least 60 semester hours (90 quarter hours) of academic credit from a regionally accredited college/university, including 6 semester hours (9 quarter hours) of chemistry and 6 semester hours (9 quarter hours) of biology AND successful completion of a 50 week U.S. military medical laboratory training course.**

**An official or notarized documentation of your military code (NEC, MOS, or AFSC code or ER.B), certificate verifying completion of training, letter from Program Director documenting program completion date, or notarized copy of your DD214 form must be submitted with your application form.

Route 2: MLT(ASCP) certification AND a baccalaureate degree from a regionally accredited college/university, including 16 semester hours (24 quarter hours) of biological science (with one semester in microbiology), 16 semester hours (24 quarter hours) of chemistry (with one semester in organic or biochemistry), one semester (one quarter) of mathematics, AND two years of full time acceptable clinical laboratory experience in Blood Banking, Chemistry, Hematology, Microbiology, Immunology and Urinalysis/Body Fluids in the U.S., Canada or an accredited laboratory* within the last ten years.

Route 4: A baccalaureate degree from a regionally accredited college/university, including 16 semester hours (24 quarter hours) of biological science (with one semester in microbiology), 16 semester hours (24 quarter hours) of chemistry (with one semester in organic or biochemistry), one semester (one quarter) of mathematics, AND five years of full time acceptable clinical laboratory experience in Blood Banking, Chemistry, Hematology, Microbiology, Immunology and Urinalysis/Body Fluids in the U.S., Canada or an accredited laboratory* within the last ten years.

Applicants may apply under the following routes. Choose the route that applies to you.
Route 1: Baccalaureate degree from a regionally accredited college/university including biological science, chemistry and mathematics courses, AND successful completion of a CAAHEP accredited Specialist in Blood Bank Technology program within the last 5 years.

Route 2: MT/MLS(ASCP) or BB(ASCP) certification, AND a baccalaureate degree from a regionally accredited college/university, AND three years of full time acceptable clinical laboratory experience in blood banking in the U.S., Canada or an accredited laboratory* within the last ten years. These three years of experience must be acquired post baccalaureate degree.

Route 3: Master’s or doctorate degree in chemistry, biology, immunology, immunohematology, microbiology, allied health, clinical laboratory sciences, or an appropriately related field, from a regionally accredited college/university, AND three years of full time acceptable clinical laboratory experience in blood banking in the U.S., Canada or an accredited laboratory* within the last ten years. These three years of experience must be acquired post baccalaureate degree.
Route 4: Doctorate degree in chemistry, biology, immunology, immunohematology, microbiology, allied health, clinical laboratory sciences, or an appropriately related field, from a regionally accredited college/university, AND two years of post-doctoral fellowship in blood banking in the U.S. or Canada within the last ten years.

*Laboratory accredited by AABB, CAP, COLA, DNV, The Joint Commission, JCI, or under ISO 15189.

Documentation
Applicants should collect the required documentation to verify their laboratory work experience and academic education as follows:

1. If your program is CAAHEP accredited (Route 1) - contact your Program Director for the school code number. This number must be indicated on the application form. An official college transcript verifying a baccalaureate degree must be submitted following graduation to the ASCP BOC office at 33 W. Monroe St., Suite 1600, Chicago, IL 60603. Contact your program director if you have any questions regarding your college transcript.

2. If you are applying under the laboratory experience routes (Route 2, Route 3, Route 4) - submit an experience documentation formalong with a letter of authenticity completed by your immediate supervisor or laboratory director verifying the authenticity of this form. This letter of authenticity must be printed on original letterhead and state that the experience documentation form was completed by the employer, with the date and the employer’s signature. These documentation forms and letters of authenticity should be submitted with the application form, if you are submitting a printed application. If you are submitting an online application, the documentation forms and letters of authenticity should be mailed to the address below at the same time as you submit your application.

An official college transcript verifying the appropriate degree must be submitted to the ASCP BOC office at 33 W. Monroe St, Suite 1600, Chicago, IL 60603.

Applying for Certification and Application Processing
Applying for Certification
Submit your application. The application should be completed, following the instructions on the application form. It is imperative that your personal email address is included since all information will be sent by email. Your full, complete mailing address must also be included for mailing of your certificate, if you pass the examination. It should be signed and dated. Please note: application fees are non-refundable.
- Payment by the U.S. Military by credit card - If your application fee is being paid by the military, submit your completed application form to the appropriate military personnel to be submitted to the ASCP BOC office. When payment is received from the military and authorized, processing of your application will begin. Complete processing of your application will take 30-45 days from the date payment is authorized.
- Payment by the U.S. Military by check- If the program director submits application forms for their students along with one check to cover all application fees, these applications, the check, and any accompanying documents should be mailed to the following address: ASCP Board of Certification, 3462 Eagle Way, Chicago, IL 60678. This will ensure that the payment is correctly allocated to the students’ applications.
- Personal Payment - The fastest method for submitting the application form is online with a credit card. You may choose to print the application form mail it with a check. Printed application forms with checks or money orders are mailed directly to the bank processing center (3335 Eagle Way) for deposit, and then forwarded to the BOC for review and distribution to the evaluator. Allowing for postal mail delivery, bank processing time, and delivery to the BOC office, this method may take up to 15 business days for the application to reach the BOC office. Because the bank processing center handles a high volume of mail per day from many different companies, applications should not be sent by certified mail, FedEx, UPS, or any type of delivery service except regular U.S. postal mail to avoid further delays. Complete processing of applications takes 30-45 days.
Transcripts, documentation forms, letters of authenticity or any correspondence without a check or money order should be mailed directly to the BOC office at: Board of Certification, 33 W. Monroe St., Suite 1600, Chicago, IL 60603. If sent to the bank address above, these documents will be bundled with other miscellaneous documents, and then forwarded to the BOC office.

Application Processing
Applications for certification examinations are processed immediately as they are received. As soon as the program director signs off on the form emailed to them (for current students in CAAHEP accredited programs only) the applicants will be notified to access their admission letter, barring any other issues with the application. Applicants applying under Route 2, Route 3 or Route 4 will be notified to access their admission letters as soon as all appropriate documentation, as described above, is submitted.

During peak volume times from May through August, there may be a delay in emailing admission letters, but complete applications are processed within a 30-45 day period. When eligibility has been established by the ASCP Board of Certification, the candidate will be notified of the following:
- Admission letter
- A three (3) month window of eligibility.

Examination Information
Examination Location and Scheduling
Military personnel and their families can take ASCP BOC certification examinations at military bases and any other Pearson VUE Preferred test centers. A complete listing of locations on U.S. military bases is available at www.pearsonvue.com/military. Candidates will schedule an appointment for the date and time of their choice within the three month window of eligibility, following the instructions on the admission letter. Examination Results
For students enrolled in CAAHEP accredited programs, scores will not be released until an official transcript documenting completion of the academic requirements is received in the Board of Certification office. Examination result are not released to any candidates until four business days after they take the examination. Candidates applying under the laboratory work experience routes will be notified of scores four business days after they have taken the examination.
