Eligibility
Provided they are in good standing, active military personnel who have successfully completed the CAAHEP accredited Specialist in Blood Bank Technology program and a baccalaureate degree, or have the required laboratory work experience and education may apply for certification as described below:
Applicants may apply under the following routes. Choose the route that applies to you.
Route 1: Baccalaureate degree from a regionally accredited college/university including biological science, chemistry and mathematics courses, AND successful completion of a CAAHEP accredited Specialist in Blood Bank Technology program within the last 5 years.
Route 2: MT/MLS(ASCP) or BB(ASCP) certification, AND a baccalaureate degree from a regionally accredited college/university, AND three years of full time acceptable clinical laboratory experience in blood banking in the U.S., Canada or an accredited laboratory* within the last ten years. These three years of experience must be acquired post baccalaureate degree.
Route 3: Master’s or doctorate degree in chemistry, biology, immunology, immunohematology, microbiology, allied health, clinical laboratory sciences, or an appropriately related field, from a regionally accredited college/university, AND three years of full time acceptable clinical laboratory experience in blood banking in the U.S., Canada or an accredited laboratory* within the last ten years. These three years of experience must be acquired post baccalaureate degree.
Route 4: Doctorate degree in chemistry, biology, immunology, immunohematology, microbiology, allied health, clinical laboratory sciences, or an appropriately related field, from a regionally accredited college/university, AND two years of post-doctoral fellowship in blood banking in the U.S. or Canada within the last ten years.
*Laboratory accredited by AABB, CAP, COLA, DNV, The Joint Commission, JCI, or under ISO 15189.
Documentation
Applicants should collect the required documentation to verify their laboratory work experience and academic education as follows:

If your program is CAAHEP accredited (Route 1) - contact your Program Director for the school code number. This number must be indicated on the application form. An official college transcript verifying a baccalaureate degree must be submitted following graduation to the ASCP BOC office at 33 W. Monroe St., Suite 1600, Chicago, IL 60603. Contact your program director if you have any questions regarding your college transcript.

If you are applying under the laboratory experience routes (Route 2, Route 3, Route 4) - submit an
experience documentation formalong with a letter of authenticity completed by your immediate supervisor or laboratory director verifying the authenticity of this form. This letter of authenticity must be printed on original letterhead and state that the experience documentation form was completed by the employer, with the date and the employer’s signature. These documentation forms and letters of authenticity should be submitted with the application form, if you are submitting a printed application. If you are submitting an online application, the documentation forms and letters of authenticity should be mailed to the address below at the same time as you submit your application.
An official college transcript verifying the appropriate degree must be submitted to the ASCP BOC office at 33 W. Monroe St, Suite 1600, Chicago, IL 60603.
