The ASCP Board of Certification examination for Medical Technologist (MT)/Medical Laboratory Scientist (MLS) has been approved for licensure purposes by the state of California effective October 1, 2006. The ASCP Board of Certification examinations for Technologist in Chemistry (C), Specialist in Chemistry (SC), Technologist in Microbiology (M) and Specialist in Microbiology (SM) have been approved for licensure purposes by the state of California effective March 1, 2008. ASCP Board of Certification examinations for Technologist in Blood Banking (BB), Specialist in Blood Banking (SBB), Technologist in Hematology (H) and Specialist in Hematology (SH) have been approved for licensure effective July 1, 2009. Information on the requirements and application process may be obtained at the CDPH Laboratory Field Service web site. It is your responsibility to review and complete all licensure requirements of the appropriate state.
Note: There is a four-year "look back" period, meaning that any individual who has taken either the ASCP BOR/BOC Medical Technologist, MT or Medical Laboratory Scientist, MLS certification examination after October 1, 2002 does not need to retake the examination. Individuals certified as Technologist in Chemistry, C(ASCP), Specialist in Chemistry, SC(ASCP), Technologist in Microbiology, M(ASCP) or Specialist in Microbiology, SM(ASCP) after March 1, 2004 do not need to retake the examination. Individuals certified as Technologist in Blood Banking BB(ASCP), Specialist in Blood Banking SBB(ASCP), Technologist in Hematology H(ASCP) and Specialist in Hematologist SH(ASCP) after July 1, 2005 do not need to retake the examination.

Step 2: Upon submission of the CA licensure application you will receive a letter from the state with a unique identification number. When you have been determined eligible, you will receive a qualification letter from CDPH.
Step 3: After you receive the qualification letter, complete the ASCP State Licensure Application (PDF) from the ASCP Board of Certification and include your CA unique identification number with a non-refundable application fee to the ASCP Board of Certification office (mailing instructions are in the application). If you choose to apply online for state licensure, you must use a credit card or PayPal. Application fees are non-refundable.
Step 4: You will receive an email notification regarding your admission letter with a phone number for scheduling your appointment to take the ASCP examination. Examinations are administered year-round on computer by appointment at Pearson testing sites located in California. You may schedule an appointment to take the examination at your convenience within the assigned 3 month examination period.

Step 6: Upon completion of the examination, your results (pass or fail) will appear briefly on the computer screen. Your examinee performance report will be emailed after two(2) business days. A copy of your results will also be mailed to California. The state licensure examination is for licensure purposes only. If you wish to apply for ASCP certification AND state licensure, you must complete the certification application form.
- OPTION 2: For California licensure AND ASCP certification
You must meet the eligibility requirements established by the ASCP Board of Certification and the licensure requirements established by the state of California. You must also successfully complete the ASCP certification examination administered after October 1, 2002 for MT/MLS or the ASCP certification examinations administered after March 1, 2004 for C, SC, M and SM or the ASCP certification examinations administered after July 1, 2005 for BB, SBB, H or SH.

PBT Certification Examination Application Fee: $135
ASCP Board of Certification, an approved certification organization by the California Department of Public Health, offers a certification examination in phlebotomy, Phlebotomy Technician, PBT(ASCP). Examinations are administered on computer year round by appointment. It is your responsibility to review and complete all licensure requirements of the appropriate state.

International Medical Technologist/Clinical Laboratory Scientist

The ASCP Board of Certification examination for International Medical Technologist, MT(ASCPi) has been approved for licensure purposes by the state of California effective June 1, 2007. Applicants for California licensure must first apply to California Laboratory Field Services and follow the state application process. Information on the requirements and application process may be obtained at the CDPH Laboratory Field Service web site.
- OPTION 1: Not yet certified: Applying for CA licensure and the MT(ASCPi) exam
You must meet the eligibility requirements established by the ASCP Board of Certification and the licensure requirements established by the state of California. You must also successfully complete the ASCP Board of Certification international certification examination.
Step 1: Contact the CDPH Laboratory Field Service web site for information on completing the licensure process and submit an application to California.
Step 2: Make sure you review the Eligibility requirements for International Medical Technologist, MT(ASCPi) to be sure you have met the requirements.
Step 3: To apply for ASCPi certification, complete the general Application for ASCPi Board of Certification (BOC) certification.

Submit the online application with a non-refundable application fee and all necessary documentation to the ASCP Board of Certification office. Please refer to the International Procedures for Examination and Certification for detailed eligibility requirements. Sign and submit a release form (PDF) authorizing our office to submit your examination scores to California Laboratory Field Services. This form must contain the unique California identification number provided by the state.
Step 4: Upon determination of your eligibility for examination, you will receive an admission letter via email with a phone number for scheduling your appointment to take the examination. Examinations are administered year-round on computer by appointment. Visit the Pearson website for ASCP examinations to identify a testing location that's convenient for you. You may schedule an appointment to take the examination at your convenience within the assigned 3 month examination period.
Step 5: Upon completion of the examination, your results (pass or fail) will appear briefly on the computer screen. A copy of your examinee performance report will be e-mailed to you and a copy of your results will be forwarded to California within ten (10) business days after we have received your final transcript verifying completion of the academic requirements. Upon passing the examination, you will receive a certification number on your examinee performance report and a certificate verifying your certification within 6-8 weeks.
- OPTION 2: Already certified: Obtaining CA licensure after passing the MT(ASCPi) exam
You do not need to reapply or retake the ASCP Board of Certification (BOC) certification examination.
Step 1: Contact the CDPH Laboratory Field Service web site for information on completing the licensure process and submit an application to California.
Step 2: You will need to follow the instructions under Verification of Certification to have your examination results sent to CDPH Laboratory Field Services to verify successful completion of the ASCP Board of Certification (BOC) certification examination.
Step 3: Enter your unique California identification number in the space for “ID number.”
Step 4: Choose the option to send your examination results to a third party and indicate the email address of the CDPH office provided in the letter sent to you from the California LFS Personnel Licensing Section.
